ANCILLARY BENEFITS Short Term & Long Term Disability

VOLUNTARY SHORT - TERM DISABILITY (STD) Voluntary Short - Term Disability provides a reasonable replacement of weekly income to an insured that becomes disabled for more than 7 days due to an accident or illness. All full - time employees working a minimum of 30 hours per week are eligible for Voluntary LTD coverage. The Voluntary STD benefit is equal to 50% of your weekly earnings (based salary) to a maximum benefit of $1,000 per week. VOLUNTARY STD MONTHLY RATES THIS IS AN OPTIONAL BENEFIT EMPLOYEES PAY 100% OF THE COST

VOLUNTARY LONG - TERM DISABILITY (LTD) Long - Term Disability provides a reasonable replacement of monthly income to an insured that becomes disabled for more than 6 months due to an accident or illness. All full - time employees working a minimum of 30 hours per week are eligible for LTD coverage. The LTD benefit is equal to 60% of your earnings (based salary) to a maximum benefit of $1,500 per month.
