BASIC LIFE INSURANCE Basic Life Insurance coverage provides important financial protection in the event of your death. Hunters Strategies provides eligible employees with coverage at no cost to you. All employees working a minimum of 35 hours per week are eligible and the benefit is equal to one year ’ s salary, to a maximum benefit of $100,000. ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE (AD&D) AD&D Insurance coverage provides protection in the event of accidental death, loss of hands, feet and/or vision. Hunters Strategies provides eligible employees with coverage at no cost to you. All employees working a minimum of 35 hours per week are eligible.

VOLUNTARY LIFE AND AD&D FOR EMPLOYEE AND DEPENDENTS You have the option to supplement your employer paid Basic Life and AD&D Insurance through Guardian, with Voluntary Life Insurance. You pay the cost of

the coverage and receive the benefit of a group rate. You may also elect coverage for your spouse and dependent children. Note: If you wish to enroll for the Voluntary AD&D coverage, you must also enroll for the Voluntary Life coverage.
